Late Cancellation and No-show Policy Once an appointment is scheduled, all requests for that appointment time are refused. Therefore we ask that all cancellations be made 24 hours prior to the appointment time. Forfeiture of a monthly membership appointment or gift certificate will occur for violation of this policy. A fee of $25 is due before a next appoinment can be made. A person who has been in violation of this policy three times will be asked to keep a credit card on file, which will be charged under violation of this policy. Exceptions to this policy are during the winter months when road conditions affect the safety of our clients, patients, or associates.

Membership Program

The membership program is the best offer we have available. Those who are interested commit to a 6 or 12 month membership which will entitle them to discounted services on everything the Pain Treatment & Wellness Center has to offer.

Each member will receive the following benefits during their membership term:

• Save on all bodywork prices for yourself AND allow members of your immediate family to do the same, once each -- every month.
• Bring a guest for a session at your discounted rate and get $10 credit to your account if they become a member.
• Applies to myofascial trigger point therapy as well, however all new trigger point therapy clients must pay the initial $75 or the $26 difference, based on a one year membership, if using the monthly session towards trigger point therapy. All other follow up sessions will follow membership rates.
• Gift Certificate Discounts – you get to give DISCOUNTED GIFTS! 1 hour massage for $49*. (Limit: 2/month)
• 10% off ALL Products and PTWC Sponsored Classes.  This includes ANY product we can attain for you as well.

$54/month for a 6 month commitment; or

NOTE: These fees reflect the cost of those who pay at the time service is rendered. All cases covered by a third party, such as auto insurance, work comp, personal injury, etc are subject to charges which reflect usual customary and reasonable rates (UCR).

Financial Policy

Our financial policy allows for you to enjoy the benefits of our services at competitive rates as well as utilize some options of insurance, particularly the high deductible health plans. We will accept all of the flexible spending accounts and “Benny” cards as our services are generally considered a “un-reimbursable medical expense”

Accounts you may use:

• HRA  (Health Reimbursement Account)
• HSA  (Health Savings Account)
• FSA   (Flexible Spending Account)

We will also accept automobile insurance as payment for your services, provided you were injured in an auto accident and your policy is still open and benefits are still available. In Pennsylvania, the personal injury protection (PIP) aspect of your policy covers all of your medical expenses. The Pain Treatment & Wellness Center will check your coverage and handle all of your billing so you will have to do as little as possible to enjoy great care.

Workman’s compensation insurance will also cover 100% of your therapy, provided you went through the proper channels regarding your company’s policy on injury in the workplace. Many times you will have to see the company doctor for the first 90 days. After the 90 day period is up, you may see the practitioner of your choice. The Pain Treatment & Wellness Center will check your coverage and handle all of your billing so you will have to do as little as possible to enjoy great care.
